Do not include sensitive medical records or identifiers. The eventual receiving team should delete information that is not required for the stated purpose.
Netlify may host the website and process forms. External links lead to independent organizations such as BCCOHP, AAO and Invisalign, each with its own privacy practices.
This site should not add advertising pixels, call tracking or analytics without updating this policy and obtaining any consent required by law.
Before launch, replace the placeholder owner statement with a legal entity and monitored privacy contact. Requests should then be handled according to applicable Canadian and British Columbia privacy obligations.

Set a documented retention period in Netlify or the chosen form system. Limit dashboard and notification access to authorized personnel.
No internet transmission or storage system is risk-free. Use a clinical or emergency channel supplied by the treating provider for health information.
